|
- Serves as District Representative for nursing services for assigned school
- Ensures compliance with state and federal guidelines for student health services
- Documentation of daily interactions with nurses and or other school staff
- Routine medical care:
- If a student is suggesting self-harm or harm to others, they will notify the Threat Assessment Coordinator for their school.
- If not, the student will return to class.
- Support Threat Assessment Team:
- Participate in interviews upon request of the Threat Assessment Coordinator or Behavior Interventionists
- Monitor students who are deemed a substantive risk to others until the student is removed from campus upon request of the Threat Assessment Coordinator or Principal.
- Ensure that the student is safe and directly supervised at all times during this time period.

Key Roles/Responsibilities:
- Assure compliance with procedural requirements for Threat & Risk Assessments.
- Contacted via Behavior Interventionist if mental health provider is needed and/or the student has an exceptionality of Emotionally Disturbed
- Participates in “Return to School” conference following high level threat assessment for all students with exceptionality of Emotionally Disturbed
- If needed, follow up with parents/caregivers after threat assessment to determine need to referral to school-based mental health counselor and/or outside agencies

District Level SPED Behavior Interventionist

Key Roles/Responsibilities:
- Train and support teachers in behavior intervention implementation procedures at the school level
- Initial contact for Threat & Risk Assessments once Threat Assessment Coordinator has completed Notification of Threat to Threat Assessment Team & interview of student(s)
- Complete further interviews with students and/or guardians and Threat Assessment Coordinator
- If there is a staff member who has first-hand information about the threat, they must be available to meet with the threat assessment team member.
- At least one of the student's teachers must be available to meet with the threat assessment team member.
- Once the interview is complete, coordinate Assessment Results.
- Low Level Threat- School Principal or TA Coordinator contacts parent with results
- Medium or High Level Threat- BI consults with School Psychologist or Social Worker to determine next steps for mental health supports (in addition to step A)
- For High Level threats, follow the sheriff's deputy to the hospital with necessary paperwork & remain at the hospital until parent/guardian arrives.
- Participates in “Return to School” conference following high level threat assessment as needed
- Provide a copy of all paperwork to Threat Assessment Coordinators and maintain original documentation in the Student Services Office.
- Follow up with behavior intervention plan and/or safety plan as needed
- Monitors implementation of BIP and Safety Plan
- Virtual Threat Assessments:
- Emails that have concerning searches will be forwarded to the Behavior Interventionists via the Securely program.
- The Behavior Interventionist will screen the emails and take the necessary action. If a mental health

- Adjunct member of the Behavior Team
- Contacted via Behavior Interventionist if they are not available to complete Threat Assessment due to prior duties
- If contacted by BI: Complete further interviews with students and/or guardians and Threat Assessment Coordinator
- If there is a staff member who has first-hand information about the threat, they must be available to meet with the threat assessment team member.
- At least one of the student's teachers must be available to meet with the threat assessment team member.
- Once the interview is complete, coordinate Assessment Results.
- Low Level Threat- School Principal or TA Coordinator contacts parent with results
- Medium or High Level Threat- BI consults with School Psychologist or Social Worker to determine next steps for mental health supports (in addition to step A)

Key Roles/Responsibilities (related to special education)
- Ensure that a school-based accountability system is in place to address compliance with Threat Assessment processes according to district and state guidelines (including staff training, facilitation of the crisis management plan, facilitation of both preventative and response plans to address student needs).
- Assign staff to position of Threat Assessment Coordinator
- Direct supervision of TA Coordinator
- Contact Superintendent and/or Director of Student Services regarding all medium or high level threats.
- Follow Crisis Management Plan for contacting authorities (in crisis situations)
- Seclusion and Restraint:
- Ensure documentation is completed by the personnel who monitored seclusion of the student and/or restrained the student.
- Review the documentation completed by personnel monitoring seclusion and restraint of the student.
- This documentation should be sent home to parents/guardians and to the Special Education Director within 24 hours of the incident.

Threat Assessment Coordinators

Key Roles/Responsibilities (related to special education)
- Serves as Threat Assessment Coordinator for school crisis prevention team
- Initial contact from teachers & nurses if threat assessment is believed to be needed
- Notifies principal of possible threat on campus
- Contacts parents/guardians to notify them that interviews will be completed for a threat assessment
- Request parents/guardians to report to campus to participate in interviews
- Completes student(s) and guardian(s) interview(s)
- Gathers information for behavior team:
- Threat Assessment Checklist
- Student demographic page
- Report card
- Disciplinary referrals
- Attendance record
- If sped/504, obtain folder from sped teacher/504 coordinator
- Supervise any student believed to be at risk in a designated area (private and safe location) at ALL times, until the threat assessment team arrives.
- While waiting for the threat assessment team, if the student’s behavior begins to change, he / she should be moved to a more private area while remaining under the supervision of an assigned staff member.
- Provide a private, safe, and supervised location for the threat assessment team member to conduct the threat assessment and meet with all involved parties.
- If there is a staff member who has first-hand information about the threat, they must be available to meet with the threat assessment team member.
- At least one of the student's teachers must be available to meet with the threat assessment team member.

- Schedules and participates in “Return to School” conference following high level threat assessment:
- For safety reasons, the student is NOT to be allowed back at school without a conference with the parent/guardians. This ensures a Student Safety and Intervention Plan can be completed.
- Return to school conferences should be held after 8:30 AM.
- Along with at least one member of the threat assessment team, one of the student’s teachers, and one school administrator must be present at the return to school conference.
- Confidentiality is a must.

- Ensure that school and classroom climate are positive and supportive.
- Implement Classroom management plan with fidelity
- Monitoring student engagement and interactions

- Provide any documentation of a threat to the Threat Assessment Coordinator
- Implement Behavior Intervention and/or Safety Plans

- Ensure that all Teachers/Staff (and Parents) have a copy of key IEP Information (accommodations, modifications, BIPs, services, etc.) as well as revisions that address threat behavior.
